Your request should be accompanied by the following items:<\/p>\n<p>Relevant materials from the program to which you are applying. This may be a printed paper form or an online form.<br \/>\nConfidentiality form from the program or Dickinson College (as appropriate).<br \/>\nNames, addresses, and\/or email addresses (as appropriate) where the letter should go.<br \/>\nDepending on the reason for the letter, additional materials such as your resume and personal statement may be helpful.
